Consent Forms

Before you dive everyone needs to fill in a medical consent form. If you are between 18 and 40 you can sign this yourself, however if you are not or have any medical condition, you'll need a doctor’s signature. If you are under 18 you will also need a parental consent form. You must bring these with you to the dropzone to be allowed to dive. When you sign up you will have downloaded these files, but in case you have lost them or are just curious here they are:
